Targeted Therapy
Targeted therapy involves using drugs or other substances to precisely identify and attack cancer cells while minimising harm to normal cells. This approach targets specific molecular changes that are unique to cancer cells, such as genetic mutations or protein overexpression. By interfering with cancer cell growth and survival pathways, targeted therapies can slow or stop tumour growth, shrink tumours, or prevent cancer from spreading. These treatments often result in fewer side effects compared to traditional chemotherapy, offering patients a more tailored and effective approach to cancer treatment with the potential for improved outcomes and quality of life.

About This Treatment
One of the key targeted therapies utilised by BBG is anti-HER2 therapy. HER2 (human epidermal growth factor receptor 2) is a protein that promotes the growth of cancer cells.
Approximately 20-25% of breast cancers overexpress HER2, making it a critical target for treatment. Medications such as trastuzumab (HerceptinTM), pertuzumab (PerjetaTM), and ado-trastuzumab emtansine (KadcylaTM) specifically target HER2-positive breast cancer cells, inhibiting their growth and proliferation.
Trastuzumab is a monoclonal antibody that binds to the HER2 protein on the surface of cancer cells, blocking its signalling pathways and stimulating the immune system to attack the cancer cells. Pertuzumab works in conjunction with trastuzumab by targeting a different part of the HER2 protein, thereby enhancing the blockade of HER2 signalling. Ado-trastuzumab emtansine is an antibody-drug conjugate that delivers a potent chemotherapy drug directly to HER2-positive cancer cells, minimising systemic toxicity.
In addition to anti-HER2 therapy, BBG may utilise other targeted agents based on the molecular characteristics of your tumour. For example, hormone receptor-positive breast cancers may be treated with endocrine therapy, which targets the oestrogen receptor (ER) or progesterone receptor (PR) to block the effects of oestrogen or progesterone on cancer cell growth. Common endocrine therapies include selective oestrogen receptor modulators (SERMs) such as tamoxifen, aromatase inhibitors (AIs) like letrozole or anastrozole, and selective oestrogen receptor degraders (SERDs) such as fulvestrant.
For advanced or metastatic breast cancer, targeted therapies may also include inhibitors of specific signalling pathways involved in cancer growth and survival. For example, inhibitors of the PI3K/AKT/mTOR pathway, such as everolimus, may be used in combination with endocrine therapy for hormone receptor-positive, HER2-negative breast cancer that has become resistant to standard treatments.
BBG’s approach to targeted therapy goes beyond just treating the cancer itself; it encompasses a holistic approach to patient care that addresses the physical, emotional, and psychosocial aspects of living with breast cancer. The multidisciplinary team at BBG works collaboratively to tailor treatment plans to each patient’s unique needs, preferences, and goals, with the overarching aim of improving overall well-being and quality of life.
In addition to medical interventions, BBG places a strong emphasis on supportive care services to help you manage the physical and emotional challenges of breast cancer treatment. This may include access to oncology nurses, nurse navigators, dietitians, physical therapists, social workers, and counsellors who provide education, guidance, and emotional support throughout the treatment journey.
Furthermore, BBG recognizes the importance of patient empowerment and education in promoting well-being. You will be actively involved in decision-making regarding your treatment options, and comprehensive information about the risks, benefits, and potential side effects of targeted therapies is provided to help you make informed choices about your care. Importantly, BBG’s commitment to improving well-being extends beyond the duration of active treatment. Long-term survivorship care plans are developed to support you in transitioning from active treatment to post-treatment survivorship, addressing issues such as surveillance for recurrence, management of treatment-related side effects, lifestyle modifications, and psychosocial support.
Overall, targeted therapy at the Birmingham Breast Group represents a cutting-edge approach to breast cancer treatment that combines scientific advancements with compassionate, patient-centred care. By harnessing the power of precision medicine, BBG strives to optimise outcomes and enhance your well-being when affected by breast cancer, empowering you to live your life to the fullest.
Frequently Asked Questions
Good candidates for targeted therapy at BBG include those with specific molecular characteristics in their breast cancer, such as HER2 overexpression or hormone receptor positivity. If conventional treatments have failed or you have advanced or metastatic breast cancer, you may also benefit from targeted therapy interventions tailored to your tumour biology.
Side effects of targeted therapy medications may vary depending on the specific drug used and individual factors. Common side effects may include nausea, diarrhoea, fatigue, skin rash, and risk of infection. However, targeted therapies generally cause fewer side effects compared to traditional chemotherapy.
The length of a typical targeted therapy treatment course at BBG varies depending on the specific cancer type, stage, and individual response to treatment. Treatment durations may range from several weeks to months or longer, with regular monitoring and adjustments made based on treatment efficacy and tolerability.

I have been a patient of Dr Basu since being diagnosed with Breast Cancer a few months ago. Despite the curse of my diagnosis I couldn’t have asked for a better Dr and standard of care. He and his team are super-efficient, have gone out of their way to get fast effective treatment for me and he has super-human levels of patience. During every consultation he has been ever patient, carefully listened to all my questions and concerns then taken the time to explain everything to me and provided constant reassurance. He has been accessible at all times even over weekends and the support has been broad. He looks after you as if you are family and despite having a 90 minute journey to see him at times as we wanted the fastest appointment we couldn’t ask for anything more. Our family see him as an angel not just a doctor for what he has done for us and would recommend him in less than a blink of a eye.

I first met Naren Basu in 2016 at the Queen Elizabeth Women’s Hospital. I had been diagnosed with breast cancer. He is an exceptionally skilled man at putting people at their ease and explaining everything with great patience and candour. He is a consummate professional and really knows his stuff, especially relating to breast cancer. He is very clever, witty, and a super surgeon. He carried out a reduction to both my breasts, removing the tumours at the same time. I was fortunate indeed to have my surgery via the NHS, it would have been very expensive as a private patient but worth every penny. I felt like a million dollars after my operation. I think he is a fabulous man and so skilled on so many levels and outstanding in human kindness.

Nine days before I was due to go on an extended holiday I discovered problems in both breasts. I was lucky enough to get an appointment 3 days before travel. Mr Basu even called me on the day of the appointment to ask me to come in earlier to ensure they had time to undergo all of the tests that evening. Anyone who has had these problems will know how overwhelming it is but Mr Basu took his time to speak to me and explained all of the procedures/tests that would be carried out that evening and possible outcomes. I did have a small procedure and again everything was explained fully and felt quite safe in his hands. The follow up appointments were just as thorough and Mr Basu made sure I understood all that we discussed. If I ever found myself in this position again Mr Basu would be my first port of call.

I am currently under the care of Dr Daniel Henderson at Good Hope Hospital, and I couldn’t ask for better. He listens, and speaks to you in laymans terms so you understand what is going on now and in the future. He talks to you and not at you, and has a very calm and soothing manner. If you are lucky enough to be referred to him, rest assured that you are in good hands, as he gets things actioned quickly.
